Surfactant Replacement Therapy in neonatal Respiratory Distress Syndrome; Comparative Study of Radiologic Findings with Control Group




Abstract
Purpose:
@EN To evaluate early radiographic changes of neonatal respiratory distress syndrome after surfactant treatment and comparative analysis of prognosis and complications with control group.
@ES Materials and Methods:
@EN Bovine surfactant (S-TA) treated group of 37 neonates and control group of 31 neonates were involved in this study. The criteria of cases selection were 750~2,000gm of birth weight and severer than Bomsel's stage II on initial chest
radiography. The
survival rate, mean survival days and incidence of complications of neonatal respiratory distress syndrome were comparatively analyzed with control group. After surfactant treatment, early radiographic changes and occurrence of relapse and
complications
were observed on following 3 days.
@ES Results:
@EN Surfactant treated group had a more beneficial effect in survival rate (32%) and mean survival days (8.3 days) than control group (19% and 4.8 days). As the incidence of complications, pneumothorax and pulmonary interstitial emphysemas were
more
frequently occurred in control group, but atelectasis and bronchopulmonary dysplasia occurred more often in surfactant treated group, 35 patients showed improvement in radiologic pattern to stage II or 1 and 2 patients did not changed. The
relapse
of
previous stage were developed in 6 patients, atelectasis were seen in 7 patients and pneumothorax were observed in 7 patients on following 3 days.
@ES Conclusion:
@EN The surfactant replacement treatment improved radiologic findings in early stage of neonatal respiratory distress syndrome and decreased ventilator related complications. It is necessary that close follow up radiography for the detection of
relapse
or complications.
